Below are no acceptable forms of identification:

  1. ID has expired. Image is not clear, cropped, damaged or unreadable.
  2. ID (passport) submitted does not cover signature.
  3. ID back side missing.
  4. ID does not match the person ordering the record.
  5. ID submitted mentioned as ‘Not for Real ID purposes’ or ‘Not for Federal Identification’ not accepted by the Vital Record Agency or US State Health Department, unless the state of birth matches the state of the ID.
  6. Picture with a flashlight reflection.
  1. When a Passport or Military ID is provided as your identification.
  2. When the shipping address is different from what is mentioned in your identification.

Official documents; utility bill, bank/credit statement, and etc.

  1.     Please note pieces of mail MUST be taken out of the original envelope to be accepted.
  2.     Dated within the last six months
  3.     Must be from two different sources

STATES DO NOT ACCEPT solicitation mail it must be official mail. 

Shipping labels, envelopes, and promotional mail is NOT considered official mail.
